Description: Today it takes a team of physicians and other practitioners to provide comprehensive care for a patient. The timely and accurate communication of information is frequently challenging between providers of care. These challenges to the flow of information have the potential to impact the quality of care for patients and are a major factor in claims of medical professional liability. This presentation will illuminate the key problem areas, especially those related to referrals, consultations, and transitions of care, how the patient’s perception of care may be affected and strategies to overcome the identified challenges and mitigate risk.
