Good medical record documentation supports appropriate medical care for patients and is instrumental in creating a strong defense in the event of a malpractice claim. This year, LAMMICO has created two new educational activities to help with documentation in the electronic health record (EHR).
The new courses, one online and one live lecture, introduce key attributes for EHR documentation, the “five C’s.” The “five C’s” of good medical record documentation are Correct, Clear, Contemporaneous, Civil and Complete. These courses delve into each attribute using case scenarios and information from physician interviews to provide compelling examples of both effective and deficient documentation. To help find a balance between brevity and providing sufficient patient and medical care information in EHR documentation, consider participating in the following programs:
